Registered Nurses and Licensed Practice Nurses; These professionals assess and
evaluate patients to provide a 'patient specific' plan of care under the
direction of your physician. Plans of treatment may include addressing chronic
and acute illnesses, teaching of proper care and treatment, instruction on
medication and disease processes and assistance in restoration of functions
impaired through illness.

| Home Health Aide -- |
Home Healthcare Aides/Homemakers and Personal Care Attendants Provisions are made for personal care, daily
living assistance, light housekeeping and exercising under the supervisor of the registered nurse (RN) or
licensed practical nurse (LPN) as prescribed by your physician, with the goal in mind to maintain the highest
level of function in the patient's home.

| Medical Social Services -- |
Provide for assistance with social and financial concerns, emotional support and
counseling and community resources.

| Physical Therapy -- |
Assessment and treatment utilizing rehabilitation techniques under your
physicians orders, including large motor function, gout training and exercise
techniques. The goal is restorative measures to obtain the patient's highest
level of function, strength and mobility, geared toward lower body strength.

| Occupational Therapy -- |
Assessment and treatment as ordered by your physician inclusive of small motor
function. The goal is to obtain the patient's highest level of day-to-day
activity such as feeding, grooming and daily living tasks and to increase
overall upper body strength.

| Speech Therapy -- |
Assessment and treatment as ordered by your physician inclusive of speech and
swallowing disorders. Plans of care are designed to optimize communicative
effectiveness for clients with conditions such as stroke, voice disorders, head
injury and cognitive defects.
